Thriving Nottingham demonstrates how neighbourhood working can improve health and wellbeing when services are rooted in trusted relationships and cultural understanding. Targeted Health Coaches offer personalised support, with each session shaped around what matters most to the resident. This flexible, holistic approach allows people to lead their own journey, supported by coaches who understand their lived experience.

The Thriving Nottingham service was commissioned in a way which has allowed our team to work closely and listen to the experiences, barriers and voice of the people who live in the City. We understand that our offer might need to be shaped around people’s culture, experiences and wishes. This work is only just beginning and we are innovating with and alongside residents to ensure our offer is as equitable and accessible as possible.”

Thriving Nottingham uses an evaluation framework that blends Key Performance Indicator data with real‑life feedback. The service measures changes in mood, sleep, confidence, health knowledge and physical activity, while capturing case studies and staff reflections. This creates a rounded picture of impact and highlights the wider benefits of neighbourhood‑based support, including stronger links with healthcare partners and increased use of community services.

Regular and varied evaluation also ensures that quality, reach and impact are continually monitored, allowing the service to make timely adaptations to processes and delivery. This responsive approach strengthens delivery and ensures that support remains relevant, effective and focuses on what individuals and neighbourhoods need.
